Authorization for School Personnel to Administer Medication to Student
Any prescription medication that will be administered at school must be accompanied by a district prescription medication form signed by the parent and physician. If a student will carry an inhaler, the same form is required with the parent and physician's signatures. ( Medication Administration Form PDF)
Allergy Action Plan Form
One of our goals in the Greendale School District is to provide a safe environment for each of our students. School District requirements for students with allergies include an Allergy Action Plan Form on file signed by the Parent and Physician / Health Care Provider. For students with Emergency Medication to be kept in school:
- All Medications are required to be labeled and in their original containers.
- Medications should be kept in the school office.
If you choose not to have an allergy action plan or medications in school for your son/daughter, the waiver form MUST be completed and returned to school. Environmental or Food Allergy Action Plan Waiver Form
Important Wellness Reminders:
- If your child has been given antibiotics, they should be on the antibiotics for 24 hours before returning to school and when you feel they can participate. Remember to have your child take ALL of the antibiotics, even after they start to feel better.
- Contagious skin rashes such as ringworm, impetigo. Follow your health care provider’s recommendations.
- Pink Eye (Conjunctivitis). It is usually recommended that a student can return to school 24 hours after treatment has started.
