To better serve our students with special diet needs due to medical needs, a Special Dietary Needs Form must be completed and signed by a licensed medical professional. All dietary orders should be handed to your school nurse on the attached form.

Parents if you put allergy information for your child into Infinite Campus, a Special Dietary Needs Form must still be filled out and handed to your school nurse.
Also, important: if your child's dietary restrictions ever change a Special Dietary Needs Form must be filled out by a licensed medical professional reversing the previous Special Dietary Needs Form. Dietary restrictions can not be removed from a student's lunch account without the updated Special Dietary Needs Form on file with the district.
