2020-21 Wellness Program Requirements
To be eligible for the 2021 HRA reimbursement ($1,750 single/$3,500 family), the following requirements must be met by December 1, 2020:
- Employee must complete a biometric screening.
- Employee (and spouse, if applicable) must complete a Vitality Health Review. The Vitality Health Review is a brief, confidential health risk assessment survey. You and your spouse will need to register for Vitality and complete the Health Review to qualify for the 2021 HRA reimbursement. Instructions on Registering for Vitality and completing the Vitality Health Review. (Both you and your spouse must do this.) Employees who do not complete the biometric screening and Vitality Health Review will not be eligible for deductible reimbursement from the District.
Biometric Screening - Employee Only
In 2020, biometric screenings are only required for employees on the health plan; spouses are not required to complete the biometric screening this year to qualify for the HRA reimbursement.
If you did not have your biometrics completed onsite, you may complete them with your own physician and then upload this form to the Vitality site. You can upload the completed form under Your Account (name in upper right corner of Vitality site) under Forms and Waivers. Or, you can email the form to wellness@powerofvitality.com or fax it to 877-224-7110. You must complete the biometric screening by December 1, 2020 to be eligible for the 2021 reimbursement.
Vitality Health Review - Employee (and Spouse if applicable)
Both you and your spouse (if applicable) must complete the Vitality Health Review questionnaire by December 1, 2020. Instructions on how to register for Vitality. As you follow the instructions, use your WEA Subscriber Number, without the -01 or -02 after the initial subscriber number for registrations. Also, you will need to click on the links for the Terms of Agreement and Privacy Policy to view the actual documents before you can click the box to accept the terms.
